Immunizations

To attend public school in Washington state, each child must present a signed CERTIFICATE OF IMMUNIZATION STATUS form showing proof of:

  1. Full immunization per the 2011-2012 Recommended Childhood Immunization Schedule.
  2. An initiation of a schedule of immunization.
  3. As of July 22, 2011, the state of Washington requires parents/guardiansof all students opting out of school vaccination requirements to show they have recieved information from a health care provider on the benefits and risks of immunizations for school.  If you are filing an exemption from immunizations, you must submit the Certificate of Exemption form with your Doctors signature when filing for an immunization exemption. This information must be presented to the school before the first day of school.
